Make a change to my medical record – new form

So that we have a clear audit trail when someone gets in touch to query an entry on their medical record, this form will now be used by anyone who wants to request a change to their medical record.

This takes effect from today – 1st November 2022.

Like our other forms, we need this form to be handed in, with proof of ID shown, so that we are sure that it’s definitely from the patient..
